Contact Information

In order that we may serve you best please keep our office updated with your most current contact information including addresses, home phone numbers, work phone numbers, cell phone numbers, emergency contact phone numbers and E-mail addresses.

Medical Information

At your initial and each subsequent visit to our office, we will review your medical and dental history.  To protect your health please advise of any changes in your medical health history, changes or additions of prescription medications, any surgeries or hospitalizations since your last visit and any other pertinent information about your health.   We require patients to complete a new medical and dental health history form once per year.

Insurance Information

In order that your dental insurance claims are processed in a timely manner please be sure to provide our office with your most current insurance information. Including insured’s names, social security number, insurance cards, and benefit plan.
